Does Medicare Cover Hearing Aids in 2026? What Californians Need to Know (Spoiler: It’s Complicated)

You’re sitting at the kitchen table in Atwater, enjoying a cup of coffee with your spouse. The birds are chirping outside, but lately, you’ve noticed you’re asking "What?" a lot more often. You’ve worked hard your whole life, paid into the system, and now that you’re on Medicare, you naturally assume your hearing health is covered.

One of our clients, let’s call her Mrs. Miller from Merced County, came to us last week with this exact situation. She had been shopping for hearing aids and was shocked to see a price tag of nearly $7,000. She looked at us with a mix of confusion and worry, asking the question many of you are asking today: "Does Medicare cover hearing aids?"

As we move through 2026, the answer remains frustratingly complex. While there have been whispers of change in Washington, the reality on the ground for Californians hasn't shifted as much as we’d hoped.

The Short Answer: Original Medicare (Part A & B) in 2026

In simple terms, the answer is no. Original Medicare does NOT cover hearing aids or the exams for fitting them.

Despite being in the year 2026, the statutory exclusions that have been in place for decades still stand. You might have heard about H.R. 500 (The Medicare Hearing Aid Coverage Act). While this bill was introduced with the hope of finally bringing hearing health into the Medicare fold, it has NOT been passed into law.

Furthermore, you may see headlines about 12 new audiology billing codes that were implemented on January 1, 2026. While these codes help doctors document services more accurately, CMS (the agency that runs Medicare) has confirmed that these remain non-payable under Original Medicare.

What Original Medicare DOES cover:

  • Diagnostic hearing and balance exams: Only if your doctor orders them to see if you need medical treatment for a non-hearing-loss condition (like vertigo or a sudden medical issue).
  • Cochlear implants: These are treated as prosthetic devices rather than hearing aids.

The California Context: Medi-Cal and Medicare Advantage

For our neighbors here in California, there are a few "safety valves" that might help, but they require careful navigation.

1. Dual Eligibility and "Share of Cost"

If you are "dual-eligible" (meaning you have both Medicare and Medi-Cal), you may have better luck. In California, Medi-Cal does offer hearing aid coverage, but it comes with a strict $1,510 annual limit.

If your income is slightly above the threshold, you might be subject to a share of cost Medi-Cal arrangement. This means you have to pay a certain amount of your medical expenses each month before Medi-Cal kicks in to help with the rest: including that hearing aid benefit.

2. Medicare Advantage (Part C)

This is where the ppo vs epo conversation often comes up. Many Californians choose Medicare Advantage plans because they frequently include "extra" benefits like dental, vision, and: you guessed it: hearing.

  • PPO Plans: Often allow you more flexibility to see specialists (like a specific audiologist) outside the network, though you might pay more.
  • EPO Plans: Usually require you to stay within a strict network but can sometimes offer lower monthly premiums.

Most of these plans in 2026 provide a "allowance" for hearing aids (often between $500 and $2,000), which significantly lowers that $7,000 price tag.

3 Things You Must Know About Hearing Coverage in 2026

  1. OTC is an Option: Over-the-counter (OTC) hearing aids are now widely available for those with mild to moderate hearing loss. These are much more affordable than prescription versions but aren't covered by Medicare either.
  2. Diagnostic vs. Routine: Never assume a "hearing test" is covered. If the purpose is to get fitted for an aid, you will likely pay 100% out of pocket under Part B.
  3. Check Your Summary of Benefits: If you are on a private plan, your coverage can change every year. Just because your plan covered hearing aids in 2025 doesn't mean the benefit stayed the same for 2026.
