Does Medicare Cover Hearing Aids in 2027? What California Families Should Know, and the Dialysis Change Landing January 1
Are you in Merced County, Atwater, or elsewhere in California wondering, “Does Medicare cover hearing aids in 2027?” You may have heard that Medicare Advantage plans offer hearing allowances, while Original Medicare does not. You may also have Medi-Cal and wonder whether it can help fill the gap.
At the same time, families managing kidney disease are hearing about proposed Medicare dialysis changes for January 1, 2027. Here is what is known, and what still needs confirmation.
Does Medicare cover hearing aids in 2027?
The short answer depends on your coverage:
| Coverage type | Hearing aid coverage |
|---|---|
| Original Medicare Parts A and B | Does not cover routine hearing aids or exams for fitting hearing aids |
| Medicare Advantage | Many plans offer hearing exams, hearing aids, or a hearing allowance |
| California Medi-Cal | May cover hearing aids for eligible adults, subject to requirements and limits |
Medicare.gov confirms that Original Medicare does not cover hearing aids or fitting exams. However, Part B may cover a diagnostic hearing or balance exam when ordered to evaluate a medical condition.
Medicare Advantage plans can add benefits that Original Medicare does not provide. Your plan may use a contracted hearing vendor, a fixed copay, or a dollar allowance. The amount, technology level, number of devices, and replacement schedule vary by plan.
The Medi-Cal path many California families overlook
If you have Medi-Cal, either by itself or alongside Medicare, ask about its hearing benefit. According to California Department of Health Care Services guidance, eligible adults may receive hearing aids when they have:
- A required prescription from an appropriate physician or ear, nose, and throat specialist
- A hearing evaluation and assessment
- An approved provider or vendor
- Prior authorization through the Medi-Cal managed care plan
The adult benefit is generally capped at about $1,510 per person per fiscal year, running July 1 through June 30. Medical necessity may allow the limit to be exceeded with additional authorization. Adult Medi-Cal generally does not cover replacement hearing-aid batteries.
This is separate from Medicare. If you have both programs, ask your county or Medi-Cal managed care plan how the benefits coordinate.

A five-step timeline to use a hearing benefit before December 31, 2026
If your plan has a 2026 hearing allowance, do not wait until the final week of December. Exams, authorizations, ordering, fitting, and billing can take time.
- Schedule the hearing exam. Confirm whether it is a diagnostic exam or a routine fitting exam, and which one your plan covers.
- Obtain the prescription or authorization. Medi-Cal and some Medicare Advantage plans may require specific documentation or approval.
- Confirm the approved vendor and remaining allowance. Ask about network rules, technology tiers, copays, and whether both ears are covered.
- Order and complete the fitting. A hearing device may require programming, adjustments, or a trial period.
- Confirm delivery and billing before December 31. Get written confirmation that the device is delivered and the claim is processed under the correct benefit year.
Unused hearing allowances usually reset on January 1 rather than rolling over. Your 2027 Annual Notice of Change (ANOC) should have arrived by September 30, 2026. Review changes to hearing benefits, providers, premiums, prescriptions, and out-of-pocket costs.
For 2027 coverage, Medicare’s Annual Election Period runs October 15 through December 7, 2026. Changes generally begin January 1, 2027.
A local example: what happened to the Alvarez family
Mr. and Mrs. Alvarez in Livingston assumed Medicare would pay for hearing aids because Mr. Alvarez’s doctor ordered a hearing test. The test confirmed hearing loss, but the routine hearing aid and fitting exam were not covered by Original Medicare.
The lesson: a covered diagnostic test does not automatically mean the hearing device is covered.
What about Medicare dialysis coverage in 2027?
Original Medicare Part B generally covers medically necessary outpatient dialysis after the Part B deductible. Patients typically pay about 20% coinsurance, and Original Medicare has no annual out-of-pocket maximum. That is why many families explore a Medigap policy or Medicare Advantage plan for additional cost protection.
The CMS CY 2027 ESRD Prospective Payment System proposed rule includes several proposed changes effective January 1, 2027:
- The home and self-dialysis training add-on could increase from $95.60 to $138.22 per training treatment.
- CMS proposes allowing that training payment during the dialysis onset period, the first 120 days, which could create a meaningful opening for patients who want to learn home dialysis.
- The proposed ESRD base rate would rise from $281.71 to $299.55 per treatment.
- Oral-only phosphate binders would be incorporated into the facility’s bundled payment structure beginning January 1, 2027, if finalized as proposed.
These are proposed figures and policies, not final guarantees. Ask your dialysis facility and Part D plan how prescriptions will be handled so you do not experience a medication gap.
Also, figures circulating for 2027 Part B, such as a $209.50 premium and $292 deductible, are projections only. CMS has not made those amounts official.
Frequently asked questions
1. Does Medicare cover hearing aids in 2027?
Original Medicare generally does not. Many Medicare Advantage plans may offer hearing aids or an allowance. Medi-Cal may also help eligible California adults under its authorization and annual benefit rules.
2. Does Medi-Cal cover hearing aids?
Yes, Medi-Cal may cover hearing aids for eligible adults when required evaluations, prescriptions, approved providers, and authorization rules are met. The general adult cap is about $1,510 per fiscal year, with possible medical-necessity exceptions.
3. Will my Medicare Advantage hearing allowance roll over?
Usually, unused allowances do not roll over into the next benefit year. Check your plan documents and confirm the deadline with the plan’s hearing vendor.
4. Does Medicare cover dialysis?
Yes. Original Medicare generally covers medically necessary dialysis, but Part B coinsurance can apply and Original Medicare has no annual out-of-pocket maximum.
5. What changes for phosphate binders in 2027?
The proposed ESRD rule would incorporate oral-only phosphate binders into the facility bundled payment beginning January 1, 2027. Because the rule is not final, confirm billing and prescription instructions with your facility and Part D plan.
